The Icepick Surgeon by Sam Kean: When Science Loses Its Ethics

Sam Kean’s The Icepick Surgeon is a collection of stories from the history of science, focusing on people whose work sits somewhere between ambition, moral blindness and outright harm.

In tone and structure, it recalls books such as John Gribbin’s Science: A History and Bill Bryson’s A Short History of Nearly Everything, although Kean’s scope is narrower and his approach more closely resembles true crime. Gribbin and Bryson use scientists and their personalities to explain the development of knowledge. Kean begins with the personalities—and with what they were willing to do.

The Mad Scientist the Book Does Not Quite Deliver

The book is organised into twelve chapters around different forms of misconduct, ranging from piracy and slavery to animal cruelty, ambition, espionage, torture, malpractice and fraud. Each chapter uses one or more historical figures to explore what happens when the pursuit of knowledge becomes entangled with vanity, power or indifference to suffering.

That structure immediately invites the familiar mad-scientist image: megalomaniacal brilliance, dangerous ideas and intellect liberated from ethical restraint. It is an enjoyable trope, but it is not really what Kean’s subjects represent.

Many are disturbing precisely because their motives are ordinary. They want recognition, professional advancement, money, data or the satisfaction of being proved right. Some present themselves as benefactors. Others accept the assumptions of their institutions so completely that the human cost disappears from view.

The result is less a history of brilliance run amok than an account of how ordinary human weaknesses become dangerous when combined with authority and weak oversight. The road to hell may be paved with good intentions, but professional prestige and convenient self-deception appear to provide much of the labour.

Storytelling and the Limits of the Framework

Kean’s narrative style is one of the book’s strengths. He is an effective storyteller, and he weaves disparate lives and periods into a clear through-line despite the episodic structure.

That accessibility may also explain why I still prefer Gribbin or Bryson overall. Their broader histories create a stronger sense of ideas developing across centuries. Kean’s chapters are more immediate and character-driven, but the scientific history can occasionally become secondary to the crime, scandal or personality.

This is not always a weakness. The human stories are often what make the ethical problem visible. But it creates tension with the book’s promise: some chapters illuminate the corrupting relationship between science and power, while others feel like compelling historical crimes that happen to involve scientists.

Moral Judgement and Historical Explanation

One recurring difficulty is the tone. At times, the prose almost reeks of moral indignation. The reaction is understandable; much of the material is horrifying. Condemnation, however, is not always the same thing as explanation.

The problem is not simply that Kean judges historical figures by modern standards. Slavery, deception, coerced experimentation, reckless surgery and professional fraud were not ethically uncontested even in their own periods. Victims objected, contemporaries criticised, and perpetrators often concealed what they were doing precisely because they understood that honest disclosure would bring resistance.

The inconsistency lies elsewhere. In some chapters, Kean carefully reconstructs the incentives, institutions and assumptions that allowed abuse to continue. In others, the moral verdict arrives before the surrounding machinery has been examined with the same attention.

The book is at its strongest when indignation leads to analysis: not merely asking how someone could do something terrible, but why colleagues, universities, laboratories, governments or professional organisations allowed it to appear legitimate.

The Chapters That Work Best

The opening chapter on piracy, centred on William Dampier, is among the most interesting. Dampier was a buccaneer, explorer and acute observer of the natural world. Scientific curiosity and predation do not alternate neatly in his life; they coexist.

That makes him an effective opening figure. He cannot be reduced to either heroic naturalist or criminal opportunist. The knowledge he gathered was made possible partly by the violent and exploitative life that carried him around the world.

The chapter on slavery is important for similar reasons. It examines natural history produced within the economy of the transatlantic slave trade. I found it somewhat less successful because the scientific relationship occasionally receded behind the broader history of slavery, but the underlying point remains valuable: knowledge is never collected outside the economic and political systems that make collection possible.

The murder chapter is engaging, although it reads more like true crime than an examination of scientific ethics. The espionage chapter, centred largely on Harry Gold and the Manhattan Project, has the same problem. Gold’s psychology and tragic self-deception are fascinating, but the connection to scientific malpractice is more indirect.

A brief turn towards Lysenkoism brings the central subject back into focus. There the corruption of science is institutional and explicit: political power determines acceptable conclusions, while evidence becomes dangerous when it contradicts ideology.

Lobotomy, Torture and the Authority to Harm

The lobotomy material is probably the most chilling section of the book. Egas Moniz and Walter Freeman were not merely experimenting with an uncertain treatment. They were operating in a culture that rewarded intervention, confidence and dramatic results while giving patients little power to refuse.

My recent reading about consciousness and materialism made these chapters especially unsettling. The brain can survive extraordinary physical disruption, but survival is not the same as preservation of personality, agency or subjective life. The casual certainty with which irreversible damage was inflicted in the name of progress is difficult to comprehend.

The chapter on torture, involving Harvard psychologist Henry Murray and Ted Kaczynski, is also disturbing but less structurally successful. Murray’s experiments raise serious questions about consent, psychological harm and the confidence of mid-century behavioural science.

Kaczynski inevitably becomes the more compelling figure, however, and his later biography threatens to overwhelm the narrower question of Murray’s research ethics. The experiments should not be treated as a simple explanation for the Unabomber’s crimes. The chapter is strongest when examining the researchers’ assumptions, not when inviting speculation about a direct causal path from subject to terrorist.

Recent Failures and Familiar Institutions

The final chapters on malpractice and fraud move into more recent history. Their subjects are less remote and therefore easier to recognise, although that familiarity also makes them less surprising.

These chapters nevertheless strengthen the book’s larger argument. Scientific misconduct does not belong to a barbaric past from which modern institutions have cleanly escaped. It continues wherever prestige, productivity or institutional convenience matter more than scrutiny.

Recent cases also make it harder to blame ignorance alone. Modern laboratories and medical institutions often possess formal ethical rules, professional standards and oversight mechanisms. Harm persists because those systems can be ignored, manipulated or subordinated to organisational self-protection.

The Ordinary Face of Bad Science

The Icepick Surgeon is an engaging and frequently disturbing book, held together by Kean’s strong storytelling and clear narrative voice. Its organising theme sometimes stretches too far, and the moral framing can be uneven.

Its central lesson is nevertheless stronger than the mad-scientist premise used to package it. The danger does not come only from brilliant monsters pursuing forbidden knowledge. It comes from ordinary ambitions being granted scientific authority and protected from consequences.

Science is a method for investigating the world, but it is also a human institution. It depends on incentives, hierarchies, professional reputations and systems of accountability. When those systems fail, intelligence does not provide moral protection. It may merely make the harm more efficient.

The book promises tales of rogue scientists. What it ultimately reveals is more unsettling: wrongdoing rarely needs a laboratory villain laughing over a forbidden experiment. Sometimes it needs only confidence, permission and no one willing to say stop.
